Physical/Health Disabilities Itinerant Program
Intermediate District 287's Physical/Health Disabilities Itinerant program is committed to providing customized services to member and non-member school districts including a licensed Physical/Health Disabilities (P/HD) teacher who is specialized in educating students in grades Pre-Kindergarten through Transition who have a medically diagnosed physical impairment, an acute/chronic health disability, or a traumatic brain injury.
Minnesota Rule requires a licensed Physical/Health Disabilities teacher to be involved in evaluation and educational program planning with all students who are medically diagnosed and qualify for special education services under the category of Physically Impaired.
The team may want to consider involvement of a Physical/Health Disabilities teacher for students with acute/chronic health disability and traumatic brain injury. The Physical/Health Disabilities teacher, as a member of a coordinated team, could identify and address complex learning styles and educational needs in a continuum of services which maximizes student potential.
Services provided by a P/HD teacher:
- Initial and Re-evaluation
- Review of medical documentation that is required for the special education categories of Physically Impaired, Other Health Disabilities and Traumatic Brain Injury.
- Assisting the school's evaluation team by utilizing formal and informal evaluation tools that determine the degree in which a student's medical diagnosis impacts his/her functioning in the school environment.
- Interpretation of Minnesota's special education criteria in the areas of Physical Impaired, Other Health Disabilities and Traumatic Brain Injury.
- Individual Education Plan
- Assist team in IEP development that reflects students' educational needs:
- Present levels of performance
- Goals and objectives
- Accommodations
- Documentation of service delivery
- Implementation of IEP and development of appropriate programming
- Address assistive technology
- Service Delivery
- Indirect consultation
- Direct instruction
- In-service training
- Consistent involvement from a Physical/Health Disabilities teacher with student and family throughout educational years
